This position was chosen to learn about kitchen system operations within a Veterans Medical System Hospital. Duties included: dish room area; pot room area; cold food prep area (salads, sandwiches); tray line; storage area: restocked and maintained stock within date guidelines, created QA and a job position equivalent for storage area for supervisors’ consideration; delivered stock to ward patient nourishment room. Upon delivering and picking up patient trays, passed patient questions to appropriate departmental staff. Gained familiarity of present VAMC computer program outcomes to staff time expectations, and between differing intra and inter VAMC department levels staffing expectations and patient expectations, as related with the food delivery system; worked in kitchen with major equipment replacements, and in a COVID-19 environment from the kitchen perspective. Promoted customer service.

This position chosen to learn the system operations of a dietetic clerk position within the department of a private hospital setting, and as a dietetic clerk reviewed tray lines for accuracy of item selection and placement; added supplement on tray and restocked supplement-cooler for use by dietetic clerks; placed and received (via phone, computer) food item selections, and confirmed the selection against RD, computer approved food items, for patient select menus and nourishments. Reviewed patient on demand menu system new computer program area of dietetic clerks.

Employed in a geriatric in-private hospital care for nursing/hospice/respite SNIF patient nutritional medical care. Patients admitted from home/nursing home/hospice environment included pre to post surgery care; pressure ulcers; physical rehabilitation; end of life transitions, and respite care. Individualized medical nutrition care plans were created to embrace the patient and medical team chosen health decisions, which involved stabilization if surgery was performed, healing pressure ulcers, introduction of tube feedings (gastric/peg), PO meals, supplements or a combination of nutrition feeding methods. (TPN was not utilized in these patients). Supplements were adjusted for return to nursing home/home/hospice to match supplement availability/insurance coverage needs. This needed calorie counts as cross coordinated care was needed with other health care providers, the patient and family. Multi-disciplinary staff meetings were held twice weekly, to maintain quality assurance control via quarterly ward meetings. Accurate record keeping of daily clinical activities were logged as required.

Individualized medical nutritional care plans were developed for admission wards, medical and surgery wards ICU (MICU/SICU), CCU, Dialysis unit, Oncology Ward, and Psychiatric Unit. Dietitian for the ICU/Dialysis research project (patients needing nontraditional time frames re dialysis). Member of renal transplant committee. Medical nutritional care plans consisted of ordering, collecting, analyzing laboratory data; anthropometrics; medications, clinical symptoms; food related preferences/ allergies/intolerances; monitoring for alterations in appetite, swallowing or digestion; weight changes (planned/unplanned); diet history. Applied adult education principals for understanding and comprehension of prescribed diet orders and concerns of patients in nutrition classes or on ward in room. Diet orders consisted of oral (Diabetic/Renal/Heart Association Diet/Low Sodium Diet/High Fiber Diet/ tube feedings (general/specialized), TPN, or a combination as required of the former. Team member on the Nutritional Committee. Established care practice guidelines for renal patients for JCOH at Balboa Naval Hospital. Taught active-duty medical staff enteral/parental nutrition protocol for ready mission preparedness, volunteered for wellness fairs (health fairs to the community) by distributing and discussing good nutrition guidelines and practices.

On rotation for weekend/holiday coverage. Taught all medical staff and patients oral and tube feeding methods that were on assigned wards). Participated in rounds grand rounds, on assigned floors. With Ms. Protacio, MS, RD, clarified process of ordering the speech pathology test diets, for immediate on order availability of test food items in the kitchen. Dietetic Interns were guided in nutritional care practice and concepts. Encouraged Dietetic Technicians to extend learning to include supplements and tube feedings, on their assigned floors. Learned the computer system within the LBVAMC for ordering diets, tube feedings, and nourishments. Logged accurate keeping of daily clinical activities.

In consultation with the company supervisory dietitian, changed the Dietary Section of the health care plans of diabetic clients to conform to the current knowledge of Nutrition: i.e., in the Health Care Plans I emphasized the side effects of Diabetes and developed a dietary solution to deal with this aspect of the disease. Unwanted food items were substituted with more culturally relevant food items, fitting patient expectations and their medical nutrition order (okra changed to cabbage, pudding to sherbet for a diabetic).
