Implementing ISP programs twice a month to ensure person working on meeting the goals in the home. Maintain the medical records and scheduling all medical appointments documenting all in an electronic system. Meeting with persons to ensure there quality of life is up to standards. Ensuring medical appointments and community activities are being implemented as scheduled. Completed quarterly monitoring at day programs to ensure implementation of goals and monitor progress and review BSP data.
