Worked one on one, with a single consumer. and provided direct care in every aspect of their day to day life, helping ensure continuity in independent living, I assisted daily with teaching personal hygiene skills, cooking, household cleanliness, budgeting and bill pay, securing employment/job skills, medication reminders, securing doctors/necessary providers for all levels of care. Under a privately owned business, I oversaw all aspects of day to day operations. I was the contact between the customers and the business, as well as the employees and the business. I addressed and handled all calls, scheduling, complaints, orders, money management, bookkeeping, paperwork, advertising, inventory, and any other order of business the owner saw fit to ask of me.

SUPPORTED LIVING SPECIALIST BOLEY CENTERS FOR MENTAL HEALTH

OCTOBER 2001 – JUNE 2006

I maintained a caseload of approximately 12 individuals suffering with severe mental illness, typically dual diagnosis. I worked one on one with each member of my caseload, multiple times per week, some daily depending on need. I would meet with, screen and evaluate consumer in a secured setting, determining their level of readiness to be reintegrated back into the community in an independent living situation. I oversaw the transitioning from jails, group homes, etc. back into the community for independent living. I provided life skills training in, but not limited to, medication holding, handling and management, counseling, personal hygiene, money management and bill pay, securing employment and job coaching, and being the liaison between employer and consumer. I helped consumers secure all necessary resources, to allow for successful transition. I was responsible for creating a detailed treatment plan for the state per individual. This treatment plan would go to the consumers team of doctors to be signed off on; I was responsible for updating treatment plans quarterly. I worked one on one with primary care physicians and mental health providers, to ensure safety and wellbeing working with the population we served. I attended all state trainings and won the employee of the month award 3 times, in recognition of my outstanding work with my consumers.

Additional Educational Experience Belonged to a business group called DECA (Distributive Education Clubs of America). My work within the class was recognized as excellent, and I was sent to regional competitions 2 years in a row to compete in my category of, Management Decision Making Marketing. I placed 1st, my 1st year there, and 2nd, my 2nd year at state competitions.

SKILLSET SUMMARY

Detail oriented person, who works well under pressure. I can be a team player, or shine working independently. I enjoy being faced with a problem and working to find resolution. I secured the job at Boley Centers with an Associate’s degree even though it required a Bachelor’s. There were hundreds of other candidates but based on my hard work, great attitude, positive attributes, and recommendation from a college professor. I have extensive background in the restaurant industry, including supervision and management, as well as business management. I am a quick study, and eager to learn what I am unsure of.
