This is to certify that the above named individual was instructed by the City of Milwaukee Health Department not to attend Work due to a communicable disease as of 12/5/2020. This individual will be released from isolation on 12/16/2020 as long as their symptoms are improving and they have been fever-free for at least 24 hours without taking fever-reducing medication. If the individual is still experiencing symptoms on this date, they should contact the City of Milwaukee Health Department to discuss further guidance for release from isolation.
