Through sixteen years of hospital-based clinical pharmacy, I have enjoyed work- ing on a dynamic team of physicians, RNs, PT/OT therapists, and case managers. Patient Care
• Evaluate patient charts prior to daily rounds, utilizing DTM to optimize doses and eliminate duplications, as well as to ensure formulary compli- ance, antimicrobial stewardship, and pharmacokinetic–based dosing.
• Participate in daily multi-disciplinary rounds, in which I review labs, identify trends, and provide patient-specific dosing as well as medication recommendations to the medical team.
• Source medication solutions, with an emphasis on identifying and manag- ing side effects, trending labs, drug-drug interactions, and clearance issues.

• Conduct pain consultations, primarily for orthopedic and oncology patients, as well as guide management of chronic pain treatment. Consultations include one-on-one counseling with patients and family members to maximize patient buy-in and successful outcomes.
