● Clinical Dietitian responsible for the nutrition care of patients sub-acute & LTC setting
● Perform nutrition screening and assessment of residents
● Develop and implemented interdisciplinary care plans
● Provide in-service education to Food and Nutrition and Nursing staff
● Provide diet instruction to residents & families with respect to various diseases and medical conditions
● Participate in patient/resident care conferences
● Monitor patient medications and provided education with respect to food/drug interactions

● Develop and analyze menus to meet nutrition requirements of residents with varied diets

● Mentor/Precept Dietetic Interns enrolled in ADA accredited internship programs
