Skilled Acute/Subacute and Long Term Care patients. Acute, Subacute, skilled therapy, skilled maintenance, use of variety of therapy model and approach in least restrictive environment. Bedside assessment, variety of screening, standardized instrumental assessment for Dysphagia in general as well as acute/subacute patient e.g. due to respiratory pneumonia, pt intubated/on tracheostomy, NPO with Alternative G-tube/PEG tube feeding; Speech Dysarthria due to underlying neurological condition e.g. stroke which lead to poor oral motor function resulting into speech articulation, voice and Fluency disorder; TBI, Aphasia, Parkinson’s, Apraxia, Alzheimers or other type of dementia and underlying psychological conditions e.g. Anxiety, Schizophrenia; Patient with increased comorbidity secondary to multiple/complex medical diagnosis e.g. post surgical management of patient with pre-existing medical diagnosis with new onset of infection, AMS with fall and hip surgery etc. Treatment care plan, attend care plan meeting for different types of patient with varied degree of medical, psychosocial and physical functional decline.
