Reason for leaving: Gave a full 2-week notice with intent to relocate, stayed on as PRN status after 2-week notice was fulfilled due to delays in relocation. However, an immediate family members health issues prevented me from working 12 hr. shifts and so I took an 8 hr. day contract giving Covid-Vaccinations and was extended at contract. I wasn’t able to fulfill required 12 hr. PRN shifts for Parkwood during this time.
