Summary

Responsible to plan, organize, and implement a program of activity/leisure pursuits designed to meet the social, spiritual, intellectual, emotional, educational and physical needs and interests of patients in accordance with the comprehensive patients care plan. Assist with planning and coordinating activities and events, discussion groups, clubs sensory stimulation, cultural activities, religious services, special entertainment, off property trips, special events, in-house movies, instructional courses (arts and crafts), seminars, workshops, health and fitness, customized activities for all Alzheimer/Dementia and all other residents), marketing events, holiday parties, design and decorate all facilities decorations.

Prepares and distributes the monthly calendar, weekly reminders, event flyers, news letters, daily chronicle, word search and crossword puzzles, and other promotional materials needed for skilled nursing/assisted living facility.

Coordinate Resident Council and Family Meeting. Arranges special meetings for residents and families to address special interest to provide preference activities.

Purchase and inventories, supplies and equipment for activities as needed. Inspects recreation equipment to determine repair and maintenance needs and cleanliness.

Directs the movement of residents to and from on property and off property events.

Directs volunteer activities and provides training.

Maintains accurate documentation of resident’s activity attendance.

Research upcoming events and trips.

Coordinates transportation services with residents for skilled nursing facility and transportation either in house or outside vendors.

Data input of resident documentation in system.

Reports changes in residents to administrator/health service director.

Conduct activity assessments on all incoming new patients.

Create activity care plans and attend all required all care plan meeting.

Keep logbook of all psychotropic medications on residents and document any changes in behavior.

Makes daily contact visits to all one-on-one residents/patients.

I love to be creative! Very active in my family’s life I enjoy traveling and camping, I am a huge baseball freak as I spent a lot of my life supporting my son with his baseball talents from elementary school to college years as a pitcher, I am blessed with the ability to mentor other women who have been touched by breast cancer like myself, as a 20 year survivor, I am a huge advocate for a research studies conducted by John Link, who is the founder and CEO of Breastlink, where I act as a mentor I love to watch the cooking channel. My favorite time of the year is Christmas. I am a “dog freak” mother to two dogs, and Gram-Gram to three little cuties. I enjoy trying different food and love to cook, I love a good glass of wine. And enjoy HGTV and hallmark channel God and family and friends are my life! I believe in the power of prayer and I walk bye Faith!
