EMPLOYEMENT HISTORY

1. Dora Nginza Regional Hospital

(i) Current Occupation: Acting Infection Prevention and Control Coordinator From : 24/04/2020

Duties : - Gathering and analysing infection data to make evidence based decisions.

- Write IPC standard operating procedures that are evident based and in line with National/Provincial guidelines and policies to prevent transmission of Healthcare associated infections.

- Ensure that equipment and materials for IPC are available.

- Train and teach clinical and non-clinical teams in evidence-based IPC practices.

2. CAPRISA [Centre for the aids programme of research in South Africa –NGO]

- From : 2014 – 2017 Employed by CAPRISA as a Professional nurse on:

(iii) CORTIS (The Correlate of Risk Targeted Intervention Study)

(iv) CAP013 SUTHI (Addressing challenges in scaling up TB and HIV Treatment integration in Public health)

Duties: 1ST Project

- Initiated this project from the get go by referral pathways within the 11 districts of KwaZulu Natal. I was based at King Edward VIII Hospital, 922 bedded hospital providing tertiary level services making it to have a huge influx of referrals from all over KZN. I worked with Professor Yunus Moosa who is Chief Specialist and Head of Department of Infectious Diseases at the University of KwaZulu Natal, His work focuses on the care of inpatients and outpatients with complex infectious disease issues also his research interests include HIV drug resistance, diagnosis of smear negative tuberculosis and TB drug induced liver injury, that withdrew my attention as Advanced Clinical Care Registered Nurse Researcher.

- The purpose of this exercise was to profile the types of referrals received by King Edward Hospital Adult and Paediatric Infectious Disease (ID) Units. CAPRISA also wanted to determine which institutions were referring and what’s the turnaround time was before the patient can be seen at the tertiary facility.

- After data analysis, I plan to identify gaps in the current system in place by these units as well as the referral system and determine how CAPRISA could assist in making the referral system more efficient and less cumbersome to those clinicians wishing to refer patients.

- A telephonic Booking system was being utilised; whereby any clinician discusses new patients with the consultants prior to the patient being accepted. Patients are then booked by the administrator for the clinic (Infectious Disease Unit). The consultants also provide a consultative service for KZN where they discuss appropriate management and investigations for patients without taking them over. These patients are managed at their District level facilities under the guidance of the IDU team (Infectious Disease Unit). Patients deemed to be complex or severely ill requiring takeover by IDU team, are admitted via the medical intake team to the IDU/ ward.

- All new patients are reviewed by the consultant on their first visit. Prof Moosa/Dr Archary (Paeds Specialist) determines the management plan and other clinicians can review follow up patients in consultation with the Prof. & Dr Archary.

2ND IMPRESS STUDY

Duties: Professional Nurse (Supervisor)

- Making sure clinic flow runs smoothly.

- Completing TB record treatment cards, sputum results tracking to the facilities.

- Keeping data of all the patients on the study and keeping track of the sputum results according to DOH National TB Programme, and TB Daily Diary.

- I was also assisting in phlebotomy duties, doing ECG, put up intravenous infusions and observations like vitals, eye test on patients enrolled on Control Arm.

- Trained on Tier.net so that I can be aligned on Department of Health Systems.

- Full screening of newly diagnosed patients prior to treatment.

- Initiating ART and Anti TB drugs also following up on post TB Rx patients until 12 months.

- Complete all documentation data capturing and record keeping at end of each working day.

- Quality Assurance: To perform cover to cover quality of nurses' work for IMPRESS files monthly, liaise with data Quality Control Officers and respond to QC's daily.

- Ensure QA reports addressed timeously.

- Management of stock control and equipment: Ensure equipment checked weekly and documented on logs.

- To check emergency trolley and first aid kit weekly and document on logs.

- Perform weekly stock take of all clinic requirements and order stock. 3RD CAPO13 - SUTHI.

QUALITY IMPROVEMENT MENTOR

This study was about addressing challenges in scaling up TB and HIV treatment integration in Public health settings in South Africa.

Duties:

- Improvement Mentor in Department of Health facilities, for 2 days a week at uThungulu district and uGu district. Ensuring that clinical management guidelines, procedure manuals, policies, registers, standards and systems are relevant and implemented correctly. Formulated Successful Quality Improvement Teams ensuring quality assurance on HIV test kits and conducting Skills audit, especially on HIV/AIDS, TB, and ARVs considering patient's wishes, by practicing Batho Pele Principles as Proudly South Africans.

- Mentoring staff on how to follow up the exposed babies: through linkage of CCGs and outreach teams (PHC Re-engineering); Monitoring and evaluation of infection control policies to ensure Quality Assurance of Built up good team work, to rescue our country from TB, HIV/AIDS and teenage pregnancy.

4th CORTIS STUDY

- From IStOctober 2016:

- I was leading a nursing team also involved on a new Study- the Correlate of Risk Targeted Intervention Study (CORTIS).

- Duties: I made sure that the clinic flow went well everybody understands what was expected of them pertaining the Study in terms of skills and involvement if not I give spot training.

- I performed Initial Informed Consent on the main study and HIV informed consent, test for HIV, pre and post counselling, doing vitals, full screening then phlebotomy duties for COR (Correlate of Risk) and send PAXGENE to the lab.

- I was seeing participants for screening and follow up as our participant target was 3200+ per annum then follow up until 15 months (End of Study).

- Also doing Quality Control1 (QC1) on files before they went for QC2 and attending to queries daily.

- I was responsible for DOT (directly observed treatment) participants, monitoring them until completion of 12 doses each for three months if they can't make it for their clinic DOT dose used to visit those bringing doses to their work places as field visit doses.

- I was also coordinating the clinic flow inside the clinic including outreach recruitment activities. In addition to all this I screened and enrolled Cortis HR (HIV Positive) participants to the study according to protocol and SOP.

3. CORRIDOR EMPOWERMENT PROJECT (NGO)

Regional TB Reach Coordinator for KZN and Free State Duties: Targets were truck drivers, community members residing near the truck stops and Commercial sex workers.

- Screening was done in these roadside clinics and specimen sent to NHLS for GenXpert and baseline AFB's on positive screened participants.

- I conducted Basic TB training to RN and Peer Educators Conducting meetings with Department of Health in both provinces to monitor the progress of this project fortnightly as we were in partnership with them.

- Writing and dissemination of quarterly report and stats to University Reach South Africa and monthly reports and stats to CEP (Corridor Empowerment) and Department of health.

- Conducting districts and provincial meetings with DOH, organizing campaigns within the truck drivers, taxi drivers and community with the help of lay counsellors and ward councillors. My duty was to train staff and oversee the smooth running of the project with the help from DOH and NHLS.

- Duties involved sputum results tracking when positive and inform the client via cell phone of the client/patient and also with the help of Mezzanine program (Centralized system) whereby if the client was seen in KZN will be easy for a sister in Messina to follow up on the system as it will be shown even by clicking surname or identity number of the truck driver. 4. TRANSNET AGRI PORT AND BIDFREIGHT PORT OPERATIONS Professional Nurse

- Consulting clients on minor ailments, physical examinations of clients from head to toe, Medical Examinations on employment and also periodic examinations, lung functions’ test, Audiometry test, Keystone, Health Education given to employees, advised about the results if the deviation by 10% of loss of hearing noted referral to physician is advised.

Duties : All primary health care practitioner duties, involved in consultation of inmates when they are sick, monitoring the ones that are on chronic medication like asthma, epilepsy, TB/HIV, Diabetes Mellitus and hypertension doting them their treatment.

Every patient saw prescribed medication according to Essential Drug List following National guidelines.

Administration of contraceptives according to guidelines, pap smear on due times, health education and advice on choices to make, planning of children and spacing including implications. Adolescent programmes on status acceptance and importance of protecting themselves, encouraging them to be part of Love Life programmes.

Screening of patients follow up on smear positive ones, and immediately start them on Anti TB drugs and tracing of contacts and screen them also on TB.

Giving of health education, compliance and referring them to the Social workers for social grants and issue food parcels(milk, pap, bread and milk)

Monitoring of chronic conditions according to doctors’ orders. Health education given on proper diet, name of the condition, name of drugs prescribed by the doctor, adherence and importance of compliance on treatment also on the condition itself so that the symptoms can be controlled accordingly, also importance of joining support groups.
