Responsibilities: Full time staff occupational therapist at Epoch of Weston working in both short and long-term units, fully evaluating all patients upon arrival, which could be anywhere from a joint replacement to encephalopathy. Care plans are made for short term patients to return home. If long term patients are not in need of immediate services, their charts are reviewed quarterly, and a care plan is made for any decline in function. Oversight of COTAs, proper documentation throughout the episode, seeing 6-8 patients a day, attendance at interdisciplinary team care plan meetings to report on progress of therapy and arrangement of discharge recommendations, often in-home safety evaluations prior to discharge. Close monitoring of vitals throughout therapy sessions is often required. Family training of daily routines, proper equipment suggestions, and precise documentation for continuation of in-home therapy visits.
