•Responsible for monitoring their elective hip and knee patients who were part of the CMS CJR program.

•This included monthly hip and knee class teaching, pre-op phone calls, “peace of mind” facility tour, and case management of these patients until discharge.

•Worked closely and in constant communication with hospitalists and oncologists regarding oncology patients as their discharge needs could be quite complex.

•This included pre-auth for specialty meds, auth for outpatient chemo and/or radiation therapy. Responsible for daily reviews, full discharge planning, tertiary transfers, and timely signatures of IM letters.
