Meet with clients virtually or in persons who are in need for Social Security Disability benefits. Educate and explain to the client the process of applying for benefits and the eligibility requirements. Advise clients about the necessary application documentation. Assist clients in preparing an application and other required paperwork. Working with applicants Disability Determination Examiners to inquire about case status and needed medical or other case related information. Sending documents and application and other supportive records and other required paperwork to Social Security District Offices Claims Representative. Follow up with applicants’ treatment providers to respond to Social Security Disability Examiners medical records requests in a timely manner. If a client’s case is denied I would assist the applicant in preparing an appeal for their disability redetermination claim.
Disability Determination Services - Social Security Disability Examiner

Made determinations on claimant’s medical or vocational eligibility for Social Security Disability (SSDI or SSI) case. Responsible in gathering and obtaining medical information from treatment providers and claimant’s representatives. Following up with claimants to confirm case status, requesting assistance in having treating sources submit medical records and requested medical reports. Arrange for claimant to attend scheduled consultative examinations when medical records provided are insufficient to make a disability decision. When enough medical information is obtained to make a disability determination, I would present the case to medical and/or psychiatric consultant for a disability decision. I would also for possible case allowance based upon the claimant’s R.F.C. (Residual Functional Capacity) and inability to perform past or other pertinent employment.
Bedford Veterans Administration Hospital Domiciliary
Dual Diagnosis Specialist – Rehabilitation Technician

The Domiciliary Program is a 40-bed inpatient facility designed specifically to assist dually diagnosed homeless Veterans reintegrate into their respective communities. Provided support and treatment to Veterans affected by substance abuse and mental health issues. Completed intake procedures including a biopsychosocial assessment and assisted the assigned Veteran in the development of a sound individualized rehabilitation treatment plan appropriate to the needs and desires of the Veteran, and make appropriate changes as needed. I would also implement a discharge plan for the Veteran upon completion of the 6-month program. I would maintain a caseload of 6-8 veterans providing individual counseling, facilitating psycho-educational and substance abuse treatment groups.

Met with potential elderly clients to assess and to evaluate eligibility for state home care services. Developed an individual care plan designed to provide unmet needs of the elder. Developed a working knowledge of the agency, service providers, vendors, and community-based programs and became proficient to access this information. Implemented the care plan utilizing community-based service providers. Maintained the client care plan through the
monitoring of vendors service, informal supports, with clients and
readjusting care plans as needed. Maintained clients records with timely documentation of all significant communications from
