Caseload consists of 35 max clients. Conduct Case Management by assisting children with special needs locate and manage services necessary for them to live a healthy and safe life in their home. This is a component in some of the programs of the Department of Health and Hospitals. Writing and submitting detailed plan of care for each client base on their current way of life. Coordinator manages own scheduling with client on a month-to-month basis. Coordinator manages and keeps logs of materials/services paid for through Medicaid. Support Coordinator submits everything electronically. Support Coordinator files all necessary documents for clients. Support Coordinator will calculate mileage from driving to visits with clients.
