(Patients are a 92 year old with Parkinson disease and a 100 year old with a stroke and short term memory loss in their own home. Kept up with any changes and reviewed these with all staff and outside contacts within.)

Reason for Leave: The companie’s contract ended in February 2011 and from there I stayed as a stay at home mom with my daughter who was just born.
