-Responsibilities included: initially started in the office with General Outpatient Pediatrics from August 2004-April 2005 working full-time with 1 direct Pediatrician to support and provide all patients being seen that day with: appropriate greeting and asking for 2 patient identifiers before beginning completion of vital signs and reason for office visit and documented in EMR before Physician entered exam room. All exam rooms were completely cleaned, organized, completely stocked with all needed supplies should Physician need during exam and only sanitized with supplied medical wipes to ensure the safety of cross contamination. Ordered in EMF and completed all requested lab testing such as: rapid step test, rapid flu virus test, urinalysis, lead check, hemoglobin level, cholesterol testing and glucose level as any and all in-office testing were first labeled with patient’s identifying information and verified again before the lab test was completed, documented all in-office lab testing in EMR and results given to their PCP to review upon completion.. All annual well child appointments were given a completed physical form once exam completed and a copy was scanned into EMR. Administered all MD ordered immunizations and a copy of updated Immunization Form was provided to the parent before end of exam as well as per CDC website vaccine information sheets were given before immunization(s) were administered to all patients only after the approval from both the patients parent/legal guardian and Attending Pediatrician to complete the administration of any vaccine ordered. Opened in-office lab upon opening shift. and completed/documented/cleaned all lab equipment testing kit, checked and documented with initially form for each refrigerator and freezer temperature that contained any vaccines, urine/outsourced lab pick-up specimens and/or medications requiring refrigeration and reported any abnormal temperature levels directly to clinic supervisor immediately upon discovery.

In April 2005 the General Pediatric office hired a Child and Adolescent Psychiatrist, in which I was asked to be the sole Certified Medical Assistant for the Psychiatrist and then eventually the entire Child and Adolescent Behavioral Health Department which additionally included: 2 full-time Social Workers, 1 full-time Psychologist and another Part-time Psychiatrist. Providing efficient, high-quality and excellent mental health care while committing 100% support and assistance to over 300+ children, adolescents and young adults (ranging in ages from 4-24) throughout the Northwest Suburbs of Illinois. Created each individuals template and scheduled all patients for entire department. All patients were first screened and the parent/legal guardian completed this process with myself as a telephone intake to ensure that all patients were then afterwards explained the services that would best fit their child’s mental health needs and the parent agreed to begin the recommenced treatment, based upon the initial telephone intake, I would then schedule in EMR with the appropriate provider and each call was documented in EMR for provider to review before that new appointment, if preferred to receive services elsewhere, I would ensure the parent of any resource information needed before ending call. Answered and returned all daily calls/voicemails within a appropriate and timely manner, directly transferred all calls that required urgent triage to ensure safety, such as a patient with active suicidal ideation or self-harming behaviors which were advised to end call and immediately dial 911 or go to their nearest ED for immediate evaluation. Documented in EMR and completed all appropriate vital signs at time of appointment and verified no significant changes since last visit and if so, reported those changes immediately to the Physician, called in all approved prescription refills to verified pharmacies, e-subscribed refills for all non-stimulant medications and filled out all hand written prescriptions, provided to Psychiatrist for verification and signature and afterwards all hand written Rx I mailed out to verified home address of the patient or left for parent to pick up, Handled all providers incoming mail and faxes, reviewing beforehand and only sending the appropriate paperwork to providers and appropriately disposing of any/all documents according to HIPPA laws. I created templates and would modify as needed for the specific needs to each individual patient a new or updated IEP Letter or 504 Plan, beforehand each patients parent(s) received a call from myself and we reviewed the recommended accommodations per outpatient Psychiatrist/Psychologist and added any addition requested accommodations specific to each individual patient, upon letter completion and the physician reviewed/approved, letters were scanned into patients EMR and original letter with physician signature was sent, verification of completed ROI in patients EMR, to School and an additional copy to patients home address.Upon arrival to appointment and family greeted if needed parent & all patients 12y/o and up were given Release of Information forms to review, sign and then upon completion all ROIs were updated yearly and scanned into EMR. Completions of all prior-authorization requests and appeals for any psychiatric medication(s) denied by the patients insurance and notified Pharmacy, Physician and Parent upon receiving approval or if appeal denied, discussed appropriate medication change per the Psychiatrist request. Worked closely and documented telephone calls in EMR with all current and active patients: outpatient therapist/psychologists, School’s teacher(s)/psychologists, SASS, DCFS Case Manager and Therapist, primary care physicians (PCPs) and parent(s)/legal guardian to ensure the most accurate and coordination of care possible for all patients. All new patients were given, reviewed, signed a “Agreement for Treatment” form and before arrival patients EMR reviewed to ensure all documents were updated and accurate. Worked very close with other departments within the same hospital such as: Pediatric Neurology, Sleep Medicine and the Developmental Pediatrics department to provide additional services needed upon physician request and completed appropriate referral in EMR. Contacted and coordinated with Pharmaceutical Drug Representatives and requested: parent information pamphlets, co-pay discount cards and if offered cards for a free 30-Day Rx trial for most commonly prescribed psychiatric medication. Completed the training with the Genesight representative which allowed me to complete all ordering of testing materials, parent information packets, filed request for tests performed and completed the patient’s in-office portion of testing needed for both C&A Outpatient and Inpatient/PHP departments. Worked directly with the hospitals C&A Inpatient unit for a direct patient admission per patient ms Psychiatrist. Administered all injectable psychotropic prescribed medications to ensure compliance with medications prescribed by patient’s physician, all patient’s prescribed injectable medications were asked to call beforehand and a scheduled date/time with myself for administration and to ensure the safety of each patient, parent was advised by their child’s psychiatrist only a medication in its original sealed bag from pharmacy and unopened or tampered boxes would be accepted at the patient’s medication administration appointment. .
