Assess client needs and develop personal service plans by conductor monthly check-ins, Semi- Annual and Annual Reviews with a 50-60 caseload.

Assure implementation and coordination of the plan; coordination of services and therapist with families.

Oversee the development and implementation of the Individualized Family Service Plan per Act 212 And EI Regulations.

Monitor client programs as per established early intervention requirements.

Maintain records, correspondence, service plans and authorizations for each client.
