I am a Board Certified Pharmacotherapy Specialist with a Doctor of Pharmacy degree. I have experience in the area of pharmacy since 2007. My career focus has been in the areas of both retail and hospital pharmacy practice. In the retail setting I practice counseling based customer approach to ensure safe and proper use of all types of medication. Daily Verification of patient medication order entry. Ensure all medication orders entered are properly written, indicated, and safely dosed based on the entire patient profile including, labs, clinical background, and admitting diagnosis.

Perform ICU and CCU clinical rounds with medication recommendations according to clinical and up to date guidelines based on the patients presenting diagnosis and symptoms.

Comprehensive medication reconciliation reviews upon ER admission, inpatient admission, and patient discharge. Ensure patients are admitted with correct home medications, appropriate inpatient medications and a thorough review of all medications patients will be discharged home with.

Heart Failure and Stroke medication recommendations, including discharge counseling, by reviewing with the patient all important details of their new medications, relevant side effects and proper administration.

Prepare all inpatient IV admixture for any population type including, NICU, pediatrics, and elderly.

Assist in compounding NICU TPN and Biologic medications.

Assist in Rapid Response and Code calls in any unit and ensure proper medication administration

Order entry, verification and management, and prepare chemotherapy IV medications in closed system transfer devices for inpatient and outpatient oncology patients.

Participate in clinical trial management related to investigational and marketed drug product, IV admixture, proper documentation and HIPAA compliance

PACU and OR trained, and assist with surgical and anesthesia order management, clinical recommendations and post operative patient medication management.

Antibiotic Stewardship trained and assists with clinical antibiotic recommendations alongside infectious disease physicians. I review the patients’ hospital course and ensure they are managed appropriately on the correct antibiotics and any other clinical decision making regarding antibiotic medication management.
