Order DME (equipment's) as needed once discharge is confirmed. Arrange training with PT/OT/SLP and family members or caregivers for safe discharges.

Send referrals to home health services, hospice care or hospitals. Communicate the discharge planning with the physicians, nurses, business managers, MDS team and therapists.
