Care Coordinator for Cole County provides seniors in the area with information and provides services to help the individual remain living a healthy and independent life for as long as possible by attaining information match up and locate services by partnering with other local agencies to help provide these services. My day consisted of taking calls from individuals: seniors or case workers from other agencies, assessing their needs locating local resources, or setting up appointments to meet with the individual in their home or my office to assist them in filling out applications for services for example: Medicare Part D Enrolment, Social Security Benefit Planning, Medicaid, Extra Help, food stamps,energy assistant programs

( LIHEAP), and Property Tax Credits.

As the sole Care Coordinator for the county I was in charge of all daily office activity as well ordering supplies, managing phone lines, setting up appointments, keeping daily log of activities.
