Providence Country Hills Post Acute, El Cajon, CA. Director of Rehab in a 306-bed SNF with 30 rehab staff including OT’s, PT’s, SLP’s, COTA’s, PTA’s, COTA’s, rehab aids and RNA’s. Duties: payroll preparation; staffing and treatment schedule; employee performance evaluation; new employee orientation; policy and procedure representation in quarterly QA meeting; documentation training; management of ADR’s; internal documentation audits; IDT PDPM collaboration (GG scores, ICD-10 codes, case mix groups); weekly comprehensive skilled review; baseline care planning; falls screens and analysis; rehab screens; OT evaluation, treatment, discharge planning; group/ concurrent treatment coordination; resident care conferences; HMO case manager consultation; department financial metrics analysis; facility case mix group analysis; intra-facility mock compliance survey; partnership with business office and MDS for rehab invoices. Duties: collaborated w DOR to hire new therapy team for transition from contract to in-house rehab department; developed workflow for Casamba and Point Click Care required documentation; provided extensive skilled documentation training to team of OT, PT and SLP’s; management of 27 ADR’s and appeals; internal documentation audits; developed facility-specific report for weekly comprehensive skilled review; baseline care plans; falls screens and analysis; rehab screens; OT evaluation, treatment, discharge planning; group and concurrent treatment coordination, resident care conference attendance; consultation with HMO case managers; monthly billing and invoice.

Clinical assessment, delivery, fit, adjustment and repair of custom wheelchairs for MediCal, HMO and private-pay long-term-care SNF residents.
