This involved 7 years of clinical training rotating through different specialties at various hospitals attached to the University of Nottingham. At the end of my training I passed the intercollegiate specialty examination in general surgery with colorectal surgery as sub-specialty (FRCS III) Major part of training period has been spent in specialized colorectal surgical units at teaching hospitals (Prof J Scholefield, Mr N Armitage) This also included a ‘Fellowship’ in Professor Y Moriya’s Unit at the National Cancer Institute in Tokyo learning techniques of radical lymph node dissection and pelvic nerve sparing surgery in rectal cancer, pelvic exentration for recurrent rectal cancer and, Professor Kehlet’s unit at Hvidovre University Hospital in Denmark to learn Fast Track Surgery (ERAS). I performed over 30 major curative colorectal cancer resections including Laparoscopic APER, Laparoscopic total colectomy, Laparoscopic anterior resections, Lap complex diverticular disease resection, Lap reversal of Hartmann Procedure and variety of pelvic floor procedure. RESEARCH EXPERIENCE

I initiated a research project in 1997 and spent two years in looking at Matrix Metalloproteinases (MMPs) expression, both at the gene and protein level, in rectal cancer and how adjuvant pre-operative high dose radiotherapy affects the expression of MMPs and tumour biology in patients with resectable rectal cancer. Our data have suggested that pre-operative radiotherapy up-regulates MMP expression in rectal cancer, both at the gene and protein levels, although changes are more marked at the gene level. Similar effect was not seen in the adjacent normal mucosa. I presented the results at various National and International meetings. Subsequent research involving several in vitro experiments to look at the irradiation effect on tumour cell invasion through its effect on MMPs and integrins and the temporal and dose response of gelatinases from colorectal cancer cells after radiation. In vitro work has confirmed the findings of in vivo experiments and more importantly has shown that over-expression of MMPs following radiotherapy is enhanced by interaction between tumour cells and stroma. Furthermore, radiotherapy led to G2 arrest of cell cycle with parallel increase in MMP expression. Although radiotherapy led to decrease in the absolute number of viable cancer cells, the cancer cells became more invasive following radiation as compared with the control. This project is ongoing and we have planned further work to evaluate the mechanism how pre-operative radiotherapy up-regulates MMP expression in rectal cancer and to look-into prognostic factors at molecular level so that the appropriate cases of rectal cancer can be selected for pre-operative adjuvant treatment.

This project has resulted in a thesis for the degree of ‘Doctorate of Medicine’ (DM) at the University of Nottingham and several publications in various international journals.

During this project, I also coordinated Nottingham arm of Phase III Multi-Centre Placebo Controlled Trial of Marimastat in patients with Advanced Gastric Cancer. This trial was conducted by British Biotech Ltd.

During my research period, I learnt the technique of endorectal ultrasound for T staging rectal cancer and provided regional service in patients with rectal cancer. I also published two papers on this subject.

I also undertook a clinical research project, during my post graduate surgical training, to look at the efficacy of pneumatic dilatation and oesophagocardiomyotomy in patients with Achalasia Cardia. The data from this study resulted in a thesis for the degree of ‘Master of Surgery’ (MS) and publication in an international journal.
