TX. I worked on the med/surg floor as a staff nurse in this department for one year. During this time I provided individualized nursing assessment of

assigned patients, developed personalized care plans for these patients, coordinated nursing care for these patients and instructed patient and family members in care provided for patients with a variety of illness/disease processes. I cared for telemetry patients as well. After one year, in May of 1995, I transferred to the home health department where I worked initially as a field nurse. Field nurses provide nursing care in the home that includes assistance with transition from hospital to home,

extensive teaching regarding all aspects of care within their disease process/s. I provided wound care/assessment, lab work, IV infusion therapy, all aspects diabetic care, foley catheter care as well as supervision of the LVN and home health aides. I was later promoted to Clinical Case Manager. The

Clinical Case Manager coordinates scheduled skilled nursing visitis, home health aide visits, therapy visits and supervises the entire team that is responsible for providing care. The case manager is accountable for the compliance of the clinical record and the coordiation of services as well as

performance improvement monitoring. The Clinical Case Manager serves as primary liaison with Payer Case Managers and is responsible for appropriate

communication regarding patient care, response to treatment and service utilization. I worked in this position for 4 years. At that point (around August 1998) I transferred back to the hospital and worked in OB/PP for about 2 years. I cared for postpartum patients

recovering from both vaginal and c-section deliveries, instructed new mothers in all aspect of their postpartum care, all aspects breastfeeding as well as newborn care. After spending 2 years in postpartum I decided I would like the variety and autonomy of home health care again so I transferred back to home health where I worked as a field nurse and case manager again until April 2007.

Throughout my 13 years with East Texas Medical Center I frequently worked PRN on the weekends on the Med/Surg floor when extra help was needed. I was occassionally floated to ICU and ER.
