●Aug 2014-December 2014: SCO Family of Services Brooklyn, New York Health Care Integrator Assess for the need of services and develop an individual health plan; provide linkages for the child and caregiver to Waiver service providers and any additional community service needs; stabilize the environment for the child and caregiver and advocate for the child’s need.; development of a close working relationship with the child’s case planner and/or case manager; coordinate team meetings and a constellation of services and providers to meet the specific needs of each enrolled child; complete all necessary paperwork, documentation and training associated with the program in a timely fashion.

Employed as Service Coordinator, then Case Manager, and finally as one of the first Crisis Responders in the state . Became certified as a Mental Health First Aid Instructor in October of 2009 and certified 83 first responders; trained agency staff on progress note documentation. Also provided Applied Suicide Intervention Skills Training (ASIST).
