PRIMARY RESPONSIBILITIES
Merges/unmerges duplicate MRNs in EPIC and combines the actual record in paper form and/or electronic form through verification of identification, diagnosis, demographics, signatures or any identifying information available in the patients’ chart
Creates chart correction cases from the help desk fold from the in-basket in EPIC
Works all chart correction cases in EPIC, including and not limited to wrong patient documentation, correct patient wrong data, wrong manual charge, wrong encounter, missing documentation, wrong patient registered, 2 or more patients with 1 medical record number, inappropriate HARs and the same patient with 2 medical record numbers
Notifies all downstream departments/systems of data that needs corrected, as appropriate
Educates providers/staff on errors
Reviews all patient overlay resolution reports and notifies the appropriate parties of corrections needing made
Follows up on all tasks needing completed to complete corrections
Completes accounts from EPIC Waiting for Documentation and Missing Document work queues when missing documentation is received
Follow-up with ancillary/nursing departments for missing documentation as outlined
Follow-up with providers regarding missing documentation/dictation preventing the account from being coded
Sends expired physician queries back to coding at 3 days that are non-NOX queries
Add daily HIM account notes regarding follow-up status on all accounts worked
Reviews medical record documentation in electronic medical records and, creates appropriate charting deficiencies in the deficiency management system, and assigns those deficiencies to the appropriate provider(s)
Actively manages various analysis-specific work queues, electronic and manual, to ensure timely analysis and chart completion
Adheres to established company standards/policies and system workflow guidelines to add and re-assign accounts to appropriate work queues for processing
Identifies systematic problems and routes to the DT Manager for facility resolution
Meets or exceed corporate productivity standards
Meets or exceeds corporate quality standard of 95% or greater
Assists in maintaining all equipment and supplies required to provide services in an optimal manner
Promptly reports issues and trends not complying with facility or corporate policies/standards
Documents all workflows, including any alterations, modifications, and changes that will occur based on the processes that will be implemented or enhanced
Assists and coordinates in the documentation of systems policies and procedures as well as training materials
Works effectively on special project teams and meets deadlines in accordance to manager's expectations
Perform Quality Reviews
Responsible for administering quality control functions for transcription services on voice and text files to include blanks and inaccuracy data
Responsible for maintaining transcription standards, data tables and files. Responsible for researching dictation and transcription reports, as needed
Completes pended files for distribution to EMR
Makes corrections to transcribed documents
Verifies fax numbers for those maintained in EMON
Resubmits reports to EMR should the document fail or need corrections
Sends soft deletes to Manager as needed for record accuracy
Assist with external in internal customer calls related to transcription
Maintains provider roster in EMON
Reviews documents transcribed by vendor to ensure quality standards are met
Assists in cross-training other employees as necessary
Able to work independently as a remote employee
Performs other duties as assigned
