* Assist clients in applying for entitlements and obtaining identification. As needed, take clients through entitlement, medical, and mental health systems to connect with services and benefits.
* Assist clients with legal entanglements.
* Conduct intake and assist in assessment, counseling, referrals for health/social services.
* Make appointments for clients and arrange for transportation.
* Provide counseling, psycho-therapy, consultation, and crisis intervention as needed. Encourage medication adherence as appropriate.
* Provide appropriate case management services for each client which include:
Outreach / engagement / housing needs and home visits.
* Prepare a plan based on clients needs and goals for the provisions of services.
* Follow up with clients and providers of service to determine outcome of linkages.
* Provide psycho-social rehabilitation services (teaching hygiene and self-care, housing skills, budgeting, problem-solving, etc..
* Advocate on behalf of clients to ensure equity of treatment
