My role with Health Care Partners, Inc. consist of getting referrals for the company. I would set up and do Blood Pressure Screenings for seniors, to monitor their blood pressure and to call their doctor if needed. With the doctors orders we would assign staff to go out and visit patients in their home.

Working as the Program Integrity Coordinator, I was responsible for over 25 employees which include physical therapist, occupational therapist, dietician and nurses. When we received a new patient, I would make sure they were covered, with the staff, that the doctor had ordered for them. The patients would get a follow up call from me to see if they were happy with our services and if not, I would find out why and rectify the problem. I would also send out survey cards. I would monitor the staffs daily visits on the computer and they would call me at the end of the day, if there was a problem with the projected schedule. When my staff would go on vacations or need time off, I would make sure their clients were covered. I was also responsible for notifying staff for staff meeting and updates. This was an on call position, as well as working in the office. I worked as a Home Health Aide when the company needed me to. We believed in helping to keep the seniors in shape, so I would do chair exercises and relaxation with them.
