Capacity: Developed Complex Medical Patient Management Program, medical management of medically complex patients and coordination of their care. When patient stable care would revert to their PCP.

The program was unlike anything in Rochester for nurse practitioners. PCPs in clinic would refer complex medical patients or patients that were not keeping appointments for various reasons. Patients were homeless, noncompliant and needed close follow up. In addition, I would follow them in all facilities such as in-patient at hospitals, rehabs, hospice and nursing homes. I would work with patients’ families, discharge planners, MDs and social work to make sure patient was understood and that s/he understood care plans.
