Clinical:
To arrange the assessment of new clients
Facilitates the movement of the patient and family through the MS disease process by providing them with individualized education, support, and coordination of service.
Provides consultation and collaborates with physicians, nurses and other health care team members to develop a comprehensive and coordinated plan of care (Infusion center, contact center, OT, Neuro-ophthalmology …etc)
Ordering Laboratory Testing, Imaging, and Functional Testing after discussion and approval by the Most Responsible Physician. All order must be placed as per protocol with co-sign by the most responsible physician.
Support, advice and information for patients who have recently been diagnosed with MS
Ongoing support for patients with MS and their family via telephone or direct contact in clinic
assessment and advice on the management of MS, working closely with therapy colleagues within the Hospital
Education, Initiation and ongoing Management of Disease Modifying Therapies jointly with Prescribing physician
Case management for patients with complex needs
Links to other services supporting people with MS, making referrals when required

Serves as liaison with admitting office (House Supervisor) regarding hospital reservations, update admission information and cancels reservations when needed. Notifies patients of their arrival times per area workflows. Acts as a resource to all department personnel
Description of the Services provided
1.Program Coordinators provide services within the HAAD scope of practice for nurses and the requirements of their specialty area in which they have a Standard Care Arrangement (SCA) :
1.1.Multiple Sclerosis
1.1.1.Functions in collaboration with a neurologist according to a standard care arrangement. Provides nursing care to patients with multiple sclerosis (MS) .
1.1.2.The MS Coordinator assists in keeping the neurologists informed on key issues/problems of the day; ensures the neurologist has the clinical information needed to make decisions regarding patient care and treatment, including obtaining pertinent healthcare data.
1.1.3.MS Coordinator is the liaison between physician, family and patient
1.2.MS Coordinator will attend clinic visits with the neurologist for all newly diagnoses MS patients
1.2.1.MS Coordinator will coordinate all follow up visits, laboratory and imaging tests as per treatment regime and protocol.
1.2.2.MS Coordinator may discontinue specified treatments as per relevant protocol.
1.2.3.MS Coordinator assist in starting patient notes, place co-signed orders, follow up on laboratory and radiological results, communicate with other departments to ensure timeliness of obtaining results and assist in expediting consults, investigational studies, procedures
