Responsible for clinically screening patients in the Emergency room and hospital for eligibility into hospitalization at home program. Upon identifying appropriate patients, I discussed plan of care with attending physicians, consultants, RNs and patient and their family members. Once approved, I placed admission orders and facilitated therapies and nursing through our vendors. I rounded via telemedicine daily and placed orders in conjunction with collaborating Hospitalist. Once patient was appropriate for discharge, I placed appropriate orders for oral medications, home care and facilitated follow-up with PCP and other providers.
