Responsible for clinically screening patients in the Emergency room and hospital for eligibility into hospitalization at home program. Once approved, I placed admission orders and facilitated therapies and nursing through our vendors. I rounded via telemedicine daily and placed orders in conjunction with collaborating Hospitalist.
