“This position is responsible for the coordination of patient home care needs to provide home health, hospice, infusion and Home Medical Equipment (HME) services as appropriate. The HC Liaison will have a thorough understanding of Durable Medical Equipment (DME) regulations and guidelines to ensure appropriate provider approval and dispatch of equipment. She/he will deliver DME to patients; provide safety checks, education, and equipment setup while in the hospital.
The HC Liaison will review homecare services with the patient as well as complete a general home going assessment of patient needs and their home environment. She/he is responsible for maintaining strong relationships with hospital staff and reporting patient information obtained during patient discussion that may influence discharge plan.
Collaborates with external sources/agencies for patients for which OhioHealth Home Care is not able to provide services”.
