To whom it may concern,
My name is Heather Fox and I am a registered and licensed Occupational Therapist with experience in skilled nursing, home health and inpatient rehabilitation. I have gained experience within the field in discharge planning, educating clients and families regarding management of conditions and insurance reimbursement, interdisciplinary collaboration, conduction and attendance of meetings to address management of cases, and scheduling of services.
I have a passion and desire to improve the lives of others and am now seeking a wellness coordinator position to facilitate and ensure excellent care to those vulnerable populations.Though I have enjoyed my time as a clinician, I have found growth and excitement towards a new role that would allow me to coordinate care on another level.
Within my role as an Occupational therapist thus far, I have demonstrated a high level of initiative and have been recruited to fill roles within management on an as-needed basis. My organizational, communication, and leadership skills as well as my strong competency with computer systems have allowed me to excel in all positions that I have filled. My strengths include professional relationship-building, effective client and interdisciplinary communication, client satisfaction and safety, organization, punctuality, and a priority for patient-driven care. I am confident that my time spent as a clinician will allow me the ability to provide skilled care coordinator services effectively and will add value to your company. I believe I would be a great fit within your organization and I would love to be considered for the position on your team. Sincerely,
Heather A. Fox
