Carolinas Health Care System Positions- from NOV. 2005 – present:

Nov. 2005 to Sept 2011 – Blue Ridge Health Care/CHS – Fulltime Home Health Case Manager Nurse. Carried case load of appropriately 50 pts, making between 5 to 10 daily visits. Provided Skilled care in the home setting such as IVs, Wound care, Teaching, CP assessments, Labs in conjunction with other skilled needs, etc. Responsible for admissions, recerts, discharges including Oasis to meet Medicare guidelines as well as compliance with other Insurances and worked to meet JACAHO standards set by hospital for Home Health agency. Coordinated closely with all other discipline (PT, OT, ST, & HHA). Worked well with physicians to provide excellent care and continuity in the care to their home health pts. Rotated call with other RN on staff. Preceptor for new staff as needed.

Sept 2011 to May 2013- Clinical Service Coordinator –Quality – Responsible for all quality assurance of patient charting/ documentation and worked closely with audit team to improve documentation with staff education. Provided all orientation, staff education coordinator, responsible for all employee health records and annual TB testing and Fit testing for our branch. Completed all monthly reports to corporate compliance/ education dept. regarding quality for the Blue Ridge branch. Responsible for Completion of field staff nurses and therapy staff annual infection control visits for quality. May 2013 to December 2018 – Central Intake Supervisor for Healthy at Home- Supervisor over all service lines – HH, HME, HI at first, but currently only over Home Health. Supervise 18 staff members daily to provide with educational needs, and guidance as needed for referrals coming from multiple resources via fax. Our dept. handles over 3000 referrals per month. Referrals are reviewed for all needed information to process, Data entry completed, insurance verified and authorizations obtained, quality checked and assigned to one of the 6 Home health agencies under Healthy at Home. Work to coordinate with the Hospitals in areas of NC, SNFs, Rehabs, MD offices, Asst Living facilities and other referral sources to provide Home health and continuity of care for pt. in their home. Responsible for all aspects of Staff attendance and time and labor approval for payroll. Provide staff with support as needed with multiple programs our dept. works in to get patient information accurately in system for Home health to provide the care in pt.’s home. Oasis Certification 2020.

Current Position: December 2018 to Present – Clinical Services Supervisor for Health at Home Lincolnton branch - Supervisor for all clinicians in the Lincolnton Home Health branch. 16 employees: nurses, physical and occupational therapist, Medical social worker, Home health aide. Reviewed all new start of care admissions, resumption of care admissions, recertifications every 60 days, all orders and discharges in workflow setting daily. Manage daily staff issues and scheduling issues along beside the Clinical manager, payroll review and corrections. Answer calls and any issues of doctors and patients. Obtain orders and assure compliance with orders and state regulations are followed for Home Health services. Serve on leadership, quality and nursing council committees.

McDowell County Home Health, Marion N.C.– May 2004 to Nov 2005:

Fulltime Public Health Nurse – Case manager for case load of 35 to 40 patients in McDowell and Burke counties. Provided assessments and skill needs as ordered by MD included doing admission, revisits, resumptions and recerts for pts. Supervised Home Health Aides every 14 days. Home Care Management Corp- Lenoir, N.C. – Jan 2004 to April 2004:

Fulltime Nursing Supervisor- Supervised HHA making visits for (PCS services) Personal care to pts in their homes in Caldwell, Alexander, Burke, Iredell, Lincoln and Catawba counties. Jobs included client assessments, home visits, computer charting of all communication between, worker and nurse to provide continued care of clients. Responsible for hiring, in-services and evaluations of Home Health Aide employees along with other RNs. Assist other RNs with caseloads as needed.

Willow Ridge of N.C., LLC- Rutherfordton, N.C. – Sept 2002 to Dec 2003:

Staff Development Coordinator/ Nurse Manager- Assisted to manage 150 Skilled nursing facility with 150 employees. Provided orientation, all DFS/OSHA mandated in-services and ongoing training, provided all employee TB and Hepatitis vaccines as well as kept all Employee health files. Responsible for QA on all resident incidents and monthly infection reports. Made daily rounds throughout the facility and took call rotation with two other RN administrative staff. Provided hands on assistance with residents daily as needed and help with staffing issues daily.

Had one assistant to help complete admission paperwork. We alternated weekends doing admissions.
