NEPHROLOGY SOCIAL WORKER
Dialysis Social Worker: working with patient and families, primary area of concern social and psychosocial adjustment to dialysis. Which includes but is not limited to significant physical, and social economic stress/hardship, advocating for patients & families to effectively manage and overcome multiple barriers to allow for successful functioning. Further utilize resources and establish treatment plans to assist patients & families in adjusting and dealing effectively with the ongoing stress of chronic dialysis. The long-term goal is to encourage the development of the maximum level of independent social functioning.
Services Experience Includes:
Clinical Counseling (depression, anxiety, death and dying, marital and family stress, strengthening available support system, sexual dysfunction, change, or loss, of role in developmental stages of life an psychosocial functioning, issues surrounding withdrawal from dialysis and/or suicide, substance abuse/alcohol abuse).

Referrals to community resources including food banks, shelters, housing authorities, mental health, legal services, estate planning services, in-home supportive services, vocational rehabilitation, kidney support group, medical equipment supply services, etc.
Arranging for travel/transient dialysis services for patient who travel or transfer in and out of the area
Managing conflict that may occur between patient and dialysis staff. This includes behavioral problems, compliance problems and personal conflict.
Assessing and intervening in areas involving elder and dependent adults abuse and neglect
Providing education information on Advances Directives, CA POLST and durable power of attorney.
CLINICAL SOCIAL WORKER
Psychosocial Assessments to evaluate psychosocial dysfunctions and determine diagnosis & prognosis
Develop treatment plans using modalities of individual, family and group therapy as appropriate.
Collect comprehensive psychosocial and medial information to make a clinical determination for the scope of the individual’s mental health-related condition.
Provide and access services to support the need for treatment and care.
