• Manages a panel of over 500 Medicare advantage patients and over 200 Fee for Services patients. Organizes weekly meetings to plan and strategize the best care for critical patients, as well as preventative care.

• Managed a panel of 100 patients with uncontrolled diabetes over a 6-9 month period, or until Glycemic, blood pressure, and cholesterol goals were achieved. Treated 10-15 patients on full clinic days. Ordered, reviewed, and interpreted labs pertinent to DM, HTN, and CHOL. Made necessary medication adjustments as needed. Ordered discontinued medications to treat Type 2 DM, HTN and Hyperlipidemia. Educated patients on chronic and acute health conditions on a comprehensive level.
