• Manages a panel of over 500 Medicare advantage patients and over 200 Fee for Services patients. • Managed a panel of 100 patients with uncontrolled diabetes over a 6-9 month period, or until Glycemic, blood pressure, and cholesterol goals were achieved. Treated 10-15 patients on full clinic days. Ordered, reviewed, and interpreted labs pertinent to DM, HTN, and CHOL. Made necessary medication adjustments as needed. Ordered discontinued medications to treat Type 2 DM, HTN and Hyperlipidemia. Educated patients on chronic and acute health conditions on a comprehensive level.
