Discuss Advance Directives appropriately.

Educate appropriate Case Management Programs as listed above with the Member and enroll the Member to the appropriate program based on criteria met related to their current health needs.

•Coordinate with staff to find resources, coordinate with outside agencies for resources to better support and manage members.

•Coordinate services for things such as DME, Home health, outpatient therapies.

•Identify care gaps based and educate to ensure these are fulfilled based on Medicare guidelines.

Examples of Care Gaps are as follows: Medication Adherence to ACE/ARB, Diabetic, and Statin Medications, Colorectal Screening, Breast Cancer Screening, Osteoarthritis Screening, Diabetic Eye Exam, Diabetic Nephropathy Screening, and Comprehensive Medication Review with mail order Prime Therapeutics Pharmacist.

•Work closely to strengthen star ratings.

•Knowledge in geriatrics, chronic illnesses (CHF, COPD, DM, multi-disease), and transplant.
