1.Organize and administer health services and programs.

a.Give immediate treatment in case of injury or illness. In case of accident that will need an immediate hospital treatment, protocol are observed:

a.1 If the victim is Monolith Employee (Engineer, Staff and Admin Worker), Occupational Health Nurse accompany the victim to the nearest hospital or medical facility that is located not more than five (5) kilometers away from the workplace and can be reached in twenty-five (25) minutes of travel.

a.2 If the victim is Sub contractor or Trade contractor worker, their assigned Safety Officer or Timekeeper accompany their worker and take the full responsibility.

a.3 In case of serious injury, MCDC Occupational Health Nurse accompany and assist the safety officer in bringing the worker to the hospital.

b.Facilitate blood pressure monitoring of workers on the job site.

c.Facilitate the seminar on Standard Occupational First Aid, BLS CPR Training and Disaster Management Training on project site.

d.Conduct health teachings weekly on the general toolbox meeting.

e.Discuss the Policies and Programs during One-Day Safety Orientation Seminar conducted at the jobsite for newly hired workers:

-Drug-Free Workplace Policy

-Policy and Program on Tuberculosis

-HIV/AIDS Workplace Policy

-Hepatitis B Policy and Program

-Smoke-Free Workplace Policy

f.Facilitate Blood Letting Program once a year.

g.Facilitate health education and counseling on appropriate health and safety information to workers such as health hazards, habits of cleanliness and orderliness.

2.Notification and keeping of records of accidents and/or occupational illnesses.

a.Keep records regarding illnesses or accidents happened at the job site with the following demographic data: date, time, name of patient, age, occupation, contractor, and department, chief complaint with the extent and nature of accident or illnesses and the nursing interventions given.

b.Prepare and submit monthly and annual medical reports using form DOLE/BWC/HSD/oh47.

c.Prepare and submit annual medical report to head nurse using standard template.

3.Conduct inspection of sanitation conditions to detect occupational hazards and environmental conditions.

a.Inspection of canteen and barracks of workers including comfort rooms.

4.Participate in health maintenance examination

a.Assist and accompany Engineers and Staff on their Annual Physical Examinations.

b.Assist and accompany Project Managers on their Executive Check-Up.

c.Assist Company Doctor and Dentist during their site visit consultations.

5.Facilitate the HMO and Accident Insurance of Engineers, Staff, Admin Employees and Subcontractor Workers on the project site.

a.Submit lists of workers entitled for HMO and Accident Insurance at the main office.

6.In charge on implementation of preventive programs

a.Anti-flu / Influenza vaccination

b.Anti-tetanus vaccination

c.Cervical vaccination

7.Facilitate/Prepare submittals of SSS Sickness Notification Form, Accident Employees Notification Form, EC Medical Application Form, Maternity Notification Form and Paternity Notification Form of Engineers, Staff and Admin Employees.

8.Ensure coordination with the HSE Department regarding health matters and issues.

a.Attend weekly safety meeting on project sites.

b.Address to the Head Nurse all health issues and concerns on site.
