Assessments and Sentinel

SRE is rules-based expert system that transforms the medical management process from being a passive workflow into an active intelligent solution. SRE is an events processing or reactive rules engine which mimics the event driven care management model. It is based on the Event Condition Action (ECA) paradigm.SRE detects sentinel events and prevents process errors and missed opportunities. In addition, the SRE also implements rules-based predictive models.

The following Clinical packages were derived with the help of the Clinicians:

1) Disease Management Algorithms - Disease Management Identification (DMID) Ruleset in Jiva can reliably identify and systematically categorize the population that is either at high-risk of developing chronic conditions or are currently not being well-managed through the claims data (ICD9 and CPT codes). Enrolled populations can be proactively identified and managed through clinical criteria and risk assessment.

2) Assessments - A clinical assessment is a documented process that is used to evaluate, diagnose, and treat individuals. The results also can be used to improve the health and productivity of individuals through the proper care plans. These assessments provide results that only take into account the individual that is being evaluated, rather than evaluating individuals based on data that is compiled from multiple sources.

Example: Diabetes, Asthma, HF, CKD, COPD, Cancer, Pregnancy, Hypertension etc.

3) Gaps In Care measures: Gaps in Care program uses health insurance claims based on the Diagnosis and procedure codes to identify members who have missed opportunities to receive recommended medical care. Information about these opportunities is notified to the member and nurse to resolve. This analysis evaluates the improvement in clinical compliance and associated cost savings delivered by the Gaps in Care program.

Example: HEDIS GIC measures for Diabetes to identify complaint and non-complaint members

4) MCG/IQC/Healthwise: worked on the integration with the third-party tools for auto approvals and sending the education materials to members based on the disease conditions

5) Cost and Utilization Monitors: High Cost Claimant use clinical guidelines to identify and manage High Cost members. Clinical history that manifests through claims is captured to identify the members. Members are qualified for Cost & Utilization Monitors by the presence of total claim amount => 50,000$ in last one year and presence of specific disease conditions. Early detection of High Cost Claimant helps members to manage effectively. It will prevent future high cost claims.

6) Never Events: Never events are inexcusable actions in a health care setting, the "kind of mistake that should never happen". They are defined as "adverse events that are serious, largely preventable, and of concern to both the public and health care providers for the purpose of public accountability.

Example: Surgery performed on the wrong body part

7) Complex Case Management: These use clinical guidelines to identify and manage complex case members. Clinical history that manifests through claims is captured to identify the members. Members are qualified for complex case management initially by the presence of specific disease conditions and followed by identification for high risk members among those qualified as CCM.

Example: Members with multiple chronic conditions or complex conditions are identified through claims from Reference Table

Description: The HMT Hospital Management system takes care of HMT employees and other Patients in general. They also have tied up contract with customers like ISRO, Tata Housing, and BMRCL etc. HMT Hospital Management System can be broadly classified into following Modules: -

Modules Derived:

1. Patient Registration & Billing

This module takes care of complete Patient Registration and Reception Enquiries. Reception is the first point of interaction for anybody coming to the Hospital. It has all the information of the patients, doctors, departments and activities of the Hospital. All enquiries and appointments are scheduled through this module. All information available here are in real time and any enquiry about the patient status, Room Status, Doctors availability or tariff's for various services is on actual status since the data is constantly updated.

2. Administration

This Module helps in handling the overall activities of the Hospital. This helps in creating various Users and their Roles. The consultant Doctors Timing can be scheduled, and various rate lists can be created. This also helps in setting various Rates for Contract Clients for Surgery, Consultation, OPD and Other rates List.

3. Stores Inventory

This module deals with the inventory of all Hospital Equipments, Materials, Consumables, and Medicines in different departments of the hospital along with their purchase and supplier details. Requisitions for different items/equipment are sent to this store from different departments and accordingly the Central Store issues items/equipment to various departments and generate purchase orders for purchases.

4. Accounts

This module describes the overall accounts payables and receivables. This gives the description of payments made towards the consulting doctors and vendors. This covers the receipts from the customers. This also covers general payments and general receipts apart from the hospital activities.

5. Ward Management

This module deals with the overall activities of the Ward. The In-Patient medical records treated by the Nurses and Doctor are maintained. The Statement of medicines used in the ward is maintained and updated for Accounts for final billing. It also covers the Inventory which is received form stores and issued to Patients.

6. Casualty Management

This is an emergency department in the Hospital and covers all the activities of the casualty. The Patients are admitted in Casualty in case of emergency and later will be shifted to Wards. The Patients based on the Prescription given by the Doctor and the Payment receipt comes here for taking Injection, Dressing, POP, strapping etc. It also covers the Inventory which is received form stores and issued to Patients.

7. OT Management

This module deals with OT activities. Detailed breakup of operation charges, OT consumables, anesthetist charges, Operation details etc are recorded separately and updated to Accounts for Billing. This also covers the maintenance of Linen Accounts (washing of Cloths) and Gas Cylinders. A mini inventory will be maintained, and stock register will be updated based on the procurement from stores and issue to patients. This module comprises of the following components

8. Laboratory Management

This module deals with the investigation reports of the Patients. The Lab Tech will perform the test based on the prescription given by the doctor.
