-Actively employed as Hospice Medical Director covering daily average census of 130 patients in home hospice as well as coverage of inpatient hospice service at Community hospice House
-Established Transitional care program for Jencare primary care clinics: assistance with high risk medically complex patient’s appropriate and safe disposition: working with Interdisciplinary team including case managers and Social worker in designing safe care plan for such patients.
-Established preferred community outreach for patients: home health and skilled nursing/long term care facilities and home palliative care/bridge program for patients.
-Established palliative care consultation service within primary care clinic.
-Education for primary care physicians regarding integration of palliative care concepts within primary care for patients: addressing the management of chronic conditions, key geriatric syndromes, complex
symptoms management, advance care planning, discussion regarding goals of care/advance directives, and end of life care.
