Duties and Responsibilities

Provides comprehensive, coordinated care to Riverside PACE participants in day health center, nursing home or participant’s home, as appropriate. Works closely with Riverside PACE Medical Director, PCP and nursing staff to meet the medical needs of participants. Acts as physician extender per license and Riverside PACE Standard Care Agreement; following consultation with PCP, acts as spokesperson for medical director/PCP, as requested by medical director/PCP, to ensure care team is fully apprised of medical conditions of participants, hospitalizations and other medical updates. Evaluates and treats episodic illness, emphasizing promotion of health, prevention and early detection. Works closely with the interdisciplinary team.

Conducts home visits and nursing home visits

Provides relevant participant education. Educates and trains family and caregivers in appropriate care and interventions; and provides staff in-services on a variety of relevant topics.

Recommends a variety of pharmacological and non-pharmacological treatments to manage wellness and illness needs. Works closely with the MD and pharmacist.

Determines the need for and facilitating referrals to another medical specialist when deemed necessary and discusses same with Medical Director as appropriate.

Exercise judicious utilization of specialist consultations.

Participants in Quality assurance activities, systematically reviews records, treatment and plans and other sources of data, meeting timelines for providing data to QAPI Manager. Works closely with the Medical Director on special QA activities.

Assumes responsibility for maintaining competence in clinical practice. Participates in educational activities in order to maintain and increase competence. Meets and maintains eligibility requirements for certification by the professional organization and current Virginia law. Maintains professional affiliations and any required certifications.
