Resume:

To Whom This May Concern, November **, ****

My vast clinical experience both as a critical care nurse and certified Family Nurse Practitioner (FNP-BC) and my extensive teaching experience allows me the skills and knowledge to work with diverse patients and nursing students in various settings. Although I reside in California, I received my Bachelor of Science in Nursing from the University of Michigan and my Master of Science in Nursing from Michigan State University.

I have worked as an Advanced Practice Nurse and have had the opportunity to provide healthcare to various clients as a certified FNP and a Critical Care Clinical Nurse Specialist. I have worked as a FNP for Blize completing home health and hospice recertification in 2015-2016. Most currently I was working as a Nurse Practitioner completing medical disability and compensation evaluations for our veterans in San Leandro, CA until the clinical closed in December 2019. I was responsible for the advanced physical assessment, a diagnosis list, ordering labs and diagnostic tests and referrals for the veteran. This is done all in accordance with the Veterans Association requirements to assure each veteran has an accurate diagnosis list for identification of disability rating(s) and/or compensation. I also had been working for Sojourn/Bristol Hospice as a per diem FNP completing hospice recertification for our patients until this company was sold. As the FNP in these various jobs I communicated with all members of the health care team as appropriate which included patients/veterans, family members, personal physicians, physical therapists, and others. Currently I am proving

COVID-19 testing and education as a contract NP for Concentra.

My teaching experience includes teaching for many years at California State University, Channel Islands (CSUCI). I also taught as a contractual adjunct nursing faculty in the Clinical Nurse Leader (CNL) graduate program at USF from January to May 2015. I have taught theory content such as pharmacology, pathophysiology, advanced assessment and medical-surgical nursing to undergraduate nursing students. I have experience providing clinical supervision and evaluation for all levels of students in the medical-surgical areas as well as the critical care, emergency department and the simulation laboratory learning environments. I also have experience teaching returning registered nurses in the extended education program at CSUCI both in classroom instruction and online instruction. I am currently teaching at Samuel Merritt University in both the graduate and undergraduate nursing programs

I feel confident that I will be able to incorporate my extensive clinical and teaching experiences in working with patients in various clinical settings including COVID-19 testing and education. I look forward in hearing from you. The best way to contact me is through my email address adh2x8@r.postjobfree.com or via cell phone at 805-***-****.

Patricia E. Leads RN, MSN, FNP-BC, CNL
