Job Description: Initial intake and orientation to the CPSP program which consisted of a multidisciplinary team trained to meet the needs of pregnant women. This involved a thorough interview and assessment of patient needs and the establishment of care in the specific areas of nutritional, psych-social, health and education. Patients would be assigned a provider in the area of their need and appointments were scheduled with that provider. It was my job to assure that our patients utilized the services throughout their pregnancies. Prenatal classes were also provided by certified childbirth educators. Reassessments were made at each trimester and interventions were offered to the patients at each prenatal visit.
