Perform Telehealth counseling, Coordinating care for patients with MD, NP on monthly basis Conduct Telehealth IDT conferences, QAI, and monthly care plan meetings through Microsoft TEAMS platform
• Conduct nutrition assessments and provide monthly lab counseling for hemodialysis and home therapy patients

• Manage patients bone mineral outcomes through recommending and monitoring phosphate binder orders ; oral /IV Vit.D orders; and oral /IV calcimimetic orders and Renal Vitamin Supplements per MD /corporate protocols
• Collaborate with Social Worker to assist patients in obtaining coverage for phosphate binders and other meds that affect nutritional outcomes
• Collaborate and communicate with Nephrologist regarding medication changes outside of protocol.
• Achieved the goal for maintaining patients Bone Mineral Outcomes at goal range of 55% or greater in the past year of 2020 . This has kept my clinic in the top percentile of all clinics within Fresenius Medical Care.
• Maintained active participation of 60% of patients in FKC pharmacy program which has contributed to successful clinical outcomes with Bone Mineral Management
• Initiate and manage oral nutrition supplement orders as part of corporate Oral Nutrition Supplement Program
