Responsibilities:
In an effort to reduce hospital readmissions and reduce Medicare spending in skilled nursing, I developed a program to follow up with all post-acute care patients in Mercy Hospital’s preferred 12 skilled nursing facilities. Meeting with therapy / nursing departments to discuss patients’ progress, discharge plans and needs in order to develop a patient centered discharge plan / disposition. Overseeing all discharges and ordering DME and transportation as needed. Participating in Medicare and care plan conferences with patients / families, and the interdisciplinary teams to assist with reduction in length of stay. Updating social services and assigned physicians of discharge plans. Scheduling primary care physician, Home health care, and ambulatory management follow-up as necessary. Perform and document transitional care telephone call to ensure patient safety in the community. Update charts with necessary documentation.
