● Manage all candidate referrals including program intakes, discharges, certifications and timely apartment turnovers resulting in a 95% occupancy rate in compliance with contract requirement;
● Inspire, lead and supervise care team comprised of a Clinical Supervisor, Mental Health Counselor, Social Worker, Health Educator, Substance Use Counselor, and two case managers resulting in 96% average weekly engagement, coordination and monitoring of all case management activities;
● Establish and maintain close partnerships with medical and service providers, Health Homes and mobile intensive treatment teams for high-risk clients;
● Lead all quality assurance and continuous improvement activities including chart reviews, incident reporting and analysis, review of progress notes and service plans, and direct observation of client engagements;
● Rapidly and effectively develop and implement health screening measures including but not limited to temperature checks of staff, clients, visitors, and vendors to prevent COVID-19 spread;
● Initiate and maintain partnership with a health center to deliver routine on-site mobile health services that links clients who are easily lost to follow up care and treatment;
● Ensure effective and consistent communication with funder and other federal, state, and city stakeholders through monthly reporting, quality indicator data, and audits;
● Increase rent collection from 42% to 81% within a three-month period and tracked and reduced rent arrears by 40% in close collaboration with Real Estate team;

● Reported to the Vice President of Coordinated Care and was part of a multidisciplinary management team that set department priorities, improved program performance, conducted trainings, and ensured that performance metrics were achieved across 20-25 direct service providers;
● Directly managed a Ryan White funded Transitional Care Coordination program and supervised three (3) care coordinators with a combined monthly caseload of 200 clients living with HIV;
● Co-managed a Medicaid Health Homes program and ensured that quality of services met agency and Medicaid standards and supervised four (4) care managers with a combined monthly caseload of 200 Medicaid beneficiaries with chronic illnesses;
