Performed coordination of care to medical and mental health professionals to assist patients with stabilizing those life areas, while they receive MMTP to stabilize their opioid dependence.

Completed treatment plans with patient participation in a timely manner, completed opioid updates annually on OASAS website, completed psych-social updates after patients have been in treatment for one year to address any life changes. Conducted individual counseling sessions with patients on a bi-weekly and/or monthly basis to record treatment updates and to assist patients with their treatment episode. Monitor patient methadone dosage, coordinate care with patient medical and mental health physicians. Monitor patient’s submission of monthly medical and mental prescriptions.
