Follow-Me-Home Program Staff OTR/L: 1992-1994

Staff OTR involving a research project.

Implemented OT Services for an 18 month pilot study called the Follow-Me-Home Program at UPMC to decrease immediate readmissions to the hospital.

Recruited elderly clients in the hospital, and obtained needed consent forms.

Obtained consent forms, met with research team and followed protocols.

Provided OT treatment for 2-week period including case management, community-linkage, and addressing reimbursement issues for a wide range of services.
