CLINICAL ACTIVITIES SUMMARY
My major clinical activity is the supervision of medical students, residents, and fellows in the Emergency Department at Loma Linda University School of Medicine, as part of the LLU Emergency Medicine Residency Program. My clinical time consists of the evaluation and treatment of emergency patients in conjunction with LLU residents and fellows, and the main teaching activity is one-on-one bedside teaching. Study of Tomography Of Nephrolithiasis Evaluation (STONE) This was a multicenter, randomized controlled trial of ultrasonography compared with computed tomography for the evaluation of patients with suspected kidney stones. We enrolled patients at up to 17 large urban Emergency Departments (ED) and patients were randomized to one of three arms:
1) ultrasonography in the ED,
2) ultrasonography in radiology or
3) computed tomography in radiology.
We then collected precise and unbiased data on a comprehensive range of outcomes that would allow assessment of effectiveness, safety, accuracy and cost between patients randomized to one of the three groups. We included a study to classify emergency attending physicians according to their self-reported acceptance of risk, stress from uncertainty, fear of malpractice, and experience with point of care ultrasound with decisions to enroll patients into a randomized trial and to order imaging in that trial. We hypothesized that ultrasound may be a safer, more cost effective test with less radiation exposure while having similar effectiveness and accuracy as CAT scan in the diagnosis of renal stone in the setting of patients presenting to the emergency department with renal colic. Furthermore, US performed by emergency physicians may show similar characteristics as US performed by radiology. Specifically, we hypothesized that:
1) US would have similar effectiveness as CT in terms of morbidity, patient functional status, and utilization of health care resources; 2) US will be safer than CAT scan in terms of radiation exposure; 3) US will have a similar accuracy as CAT scan in the diagnosis of urolithiasis, and
4) US will have lower direct health care costs compared to CAT scan in the diagnosis of urolithiasis.
We hypothesize that validated scales characterizing emergency physician attitudes would be predictive of two decisions in the STONE trial: 1) the decision to enroll patients into the STONE trial, and 2) the decision to order additional imaging after an initial assignment to one imaging arm (CT scan vs US vs ED US), 3) the decision to obtain sub-specialty consultation.
