SKILLS:
Examine the patient’s history and then tests and measures the patient’s skills in the areas of speech, language, voice, cognition, swallowing & Functional daily activities.
Develop a plan to provide goal-directed intervention. The plan includes an intervention strategy, its purpose and its anticipated outcome.
Participate in the interdisciplinary process provides education to the patient, families and peers relating to the treatment process and to alternative treatment methods.
Emphasize safety strategies and judgment to patients and families and utilize safety techniques in practice.
Ensure that appropriate patient’s team, care-givers, and patient and family receive appropriate education and training to further the intervention process for the patient.
Provide accurate documentation of patient’s evaluations, intervention plan and updates, client/family education, team conference updates, progress notes, home exercise programs and follow up instructions.
Collaborate with the patient team, to maximize benefits of goals & improve patient functional performance & independence skills.
