HSS Clinical Coordinator for individuals with intellectual disabilities. Daily nursing review and admissions for required clinical evaluation, intervention, and collaboration of all UHC Tennessee members enrolled in the 1915c waiver including DIDD’s statewide, CAC, and Self-Determination waivers, as well as Intermediate Care Facilities (ICF). Frequent collaboration with internal departments including private duty nursing, whole person care, Choices, LTSS, and DSNP for member collaboration and resolution.
